Scanning bones with the Biophilia Intruder


3D NLS method. Scans were performed using a Biophilia Intruder with a software program that allows visualization of all bones, surrounding soft tissues, and lymph nodes. The NLS study begins with a full-body assessment and continues with a detailed assessment of the following body parts: head and neck; trunk and proximal shoulders; pelvis and proximal femur. The study protocol for each anatomical section included the following data: large-scale NLS scans in 3D in 3 mutually transverse planes; ultra-microscan patterns in the region of interest with spectral entropy analysis. The total time required for the study is approximately 30 minutes.


Bone scan. Bone scans of the skeletal system were performed in anterior and posterior views using a dual-detector gamma camera according to standard methods within 2 h after intravenous injection of 740 Mbk 99mTs-Technephore. Images were taken using low-energy high-resolution collimating optics; the entire stage was moved at 12-16 cm/min. The scanning process takes 15-20 minutes.

Focal or diffuse (homogeneous or heterogeneous) increase in the chromogenicity of a lesion by more than 4 points according to the Fleindler scale is considered a hallmark of metastatic disease during NLS imaging. Bone marrow changes are considered benign if they lie directly below the degenerative disc or close to the articular surface. Spectral entropy analysis is crucial in all cases.


Hyperspectral similarity (D<0,425) to the etalon «metastatic tumor subclone» is commonly referred to as a metastatic lesion. Validation of spectral entropy analysis results was based on histological examination of rings received after ring biopsy of bone tissue in the region of interest. The results of spectral entropy analysis were confirmed by histological methods in 99.1% of cases.


The probability of metastatic disease (when a bone scan was performed) was estimated based on the location and number of hyperactive lesions, taking into account radiopharmaceutical overfixation therein.
